maturation arrest and microdeletion and infertility

You can find a brief history of our infertility problem and applied treatments below.

Firstly, I want to inform you that we are married for 2.5 years. Unfortunately, we have not any child. Testicular biopsy was applied to my husband nearly one year ago and none of sperm was found in both testes. The pathology report showed that there was a maturation arrest (secondary spermatocyte). Then, karyotype analysis was performed and Y-chromosome microdeletion was observed. The level of his hormones (LH, FSH, and T) is in the normal range. Estradiol is higher than normal range. Meanwhile, the size of both testes is described as normal. On the other hand, there is a varicosele at the left testis and doctors suggest that it should be taken into consideration and must be attended firstly.

Consequently, we want to try our chance again in a successful clinic. But, there is inconsequence in expressions of some doctors in Turkey. Some of them do not prefer to perform second micro-TESE and therefore ICSI; some of them suggest attending varicosele before TESE; some of them want to apply drug treatment. Although, I and my husband try to recover current developments, we are very confused and do not know what we should do? Some literatures cited that spermatid injection can be applied. How much is our chance about pregnancy by spermatid injection? Also, is there any clinical study about spermatocyte injection on the world? How much is our chance to have baby?
